belt coming off?

My son's 98 4.0L throws the belt. He says bottom pulley appears out of align. Any suggestions? Says the pulley is tight with no play. We had head gsket replaced two weeks ago, replaced both idler pulleys.

might be a dumb question, but is the belt put on correctly?

this is for a 98 4.0(i have a 98 4.0 as well), the later years are different if your routing sticker is missing.

the idler pulleys and the water pump are the only ones without grooves...i wouldnt thing it could slip off the crank?
