I don't have experience with either one, and I've only seen folks here say that they would rather get the 5160's than the 5100's. From what I gathered on the Bilstein site the difference between the 5160 and the 5165 is that the former is direct fit and the latter is universal. They both seem to be of the same design and materials. So do you have custom or alternate mounting points or are you attaching them to stock mounts?

What I myself would be most concerned with would be contacting Bilstein to ensure that I got the proper valving for my setup, period.

The universal 5165's I think are valved a bit differently. Not a 100% sure though. A call to Bilstein would hopefully gain the answer. If there is a difference in the valving I would go with whichever is valved with more compliance(softer).

My thoughts on them, worth the price, Im not a big fan of the 5100's, but the 5160's are an upgrade and worth the premium. Another shock to look into is the Fox 2.0 w/reservoir as well.
