clunk in rear while driving - Help!

I have a 2001 5 speed I6. Occassionaly when i am driving i will hear a loud "clunk" in my rear end. Its like the passenger rear wheel is moving one turn slower than the driver side. This causes my jeep to make a small jerk to the right. Any ideas?

I am a new jeeper so my knowledge base is limited. I know that the PO changed the gears. Not sure if there is any correlation.
