clutch not dis-engaging

i was coming to a stop at a traffic light and my foot slipped off the clutch pedal causing the pedal to come flying foward hitting the bottom of the dash and then was just swinging back and forth. when i pushed the jeep to the side of the road and took a look i noticed the pushrod(i think its the push rod) had come out of the hole in the firewall and i also found a black foam washer on the floor. i put the washer back on and put the push rod thru the hole and depressed the clutch until it clicked which seemed to fix my problem so i then drove home home. on my way home i noticed when im in any gear and give it some gas the rpms go up as if im riding the clutch. what is causing this? someone on a different thread said that i need my clutch master cylinder replaced or is this a different issue all together? why did it come out of the fire wall to begin with?? thanks very much
