16 Pins on Timing Chain Not 15. Need Help.

Getting ready to replace the timing chain and I am finding that the timing marks on the two gears are not lining up, the cam gear is off a tooth. The engine was running fine. I did the 1 o'clock - 3 o'clock thing on the gears and there is actually 16 pins between marks not 15 as there should be. If the PO replaced the timing chain I suppose they could have reinstalled the new one wrong. Like I said, the engine runs fine, could an engine run fine with the cam gear off a tooth????
Should I remove & replace the timing chain in the same position or rotate the cam gear a tooth to get it to line up?
This is strange to me. What's the deal.
