Serpentine belt keeps shredding

Hey guys i am new here and am in need of some help. My serpentine belt keeps losing a lug or two. I thought it might be the harmonic balancer so i replaced it and it keeps happening. What else could be the problem? Thanks in advance! It is a 4.0 with 2 idler pulleys and a/c.

after it shreds, is it sitting on one pulley different than the others?
my culprit was the power steering pump out of alignment

I have gone through three belts and usually it is slipping off only the power steering and the crank pulley.

bent pully?
sharp pully edge?

mud and rocks in the belt?

Well i checked all of the pulleys. They are all aligned properly it appears. The only thing that seemed odd is the power steering pulley seemed to have been nicked by something and the edge towards the engine was bent in. So i bent it out and threw the belt back on and tightened it down more than i was before. In case i just had it too loose. We shall see how this works. Thanks for the help!
