Yoke keeps breaking

So my 1995 Yj keeps going through my rear yokes like candy. When I bought the Jeep the original one finally gave way, so I gave it another one. The next time it split the drive shaft came out as well. Now I can't seem to keep them from wearing through one of the heads on the yoke.

Any one have any ideas, maybe U joint is screwed up?

I'm out of ideas.

To Clarify, The yoke on the axle or on the T-Case?
I would assume the axle, if so are you spring over? maybe stock springs giving you a lot of axle wrap?
