(bank 1 sensor 2) 02 sensor

ok so two monthes ago i replaced a faulty 02 sensor after my cat. the code when off once i replaced it. recently the check engine came on.. i hooked it to code reader and its the same sensor.. when i clear the code it comes back after around 20 miles. Is the sensor bad again? whats going on?
