Death Wobble help?!

So the issue I have is anything from the speed of 30-55mph the wobble is not bad until I hit a bump then it shacks everything. Hard enough to take the steering from my hands and won't stop til I come to a complete stop. If I lock it into 4h I can avoid the death wobble. But can feel a vibration in the steering.
I've checked and had tires balanced.
Check ball joints and from what I can tell they are good. Will check again in the morning.
Checked hubs. Good from what I can tell.
Checked for play in steering. All good, no play.

The only thing I can see wrong is the oem track bar with a 3in track bar relocation bracket on a 4.5in lift. (I didn't do it the PO did the set up), also I'm just guessing it a 4.5in lift.
There are a few things needed done that might contribute to the DW (might contribute).
Bushings in LCA,Bad U join front right (but not very bad).

Any thoughts on what might be causing the DW?

Also forgot to mention it keeps getting worst. When it started it was only around 45-50mph and only happens with a huge bump or series of medium bumps. Now it's 30-55mph and happens every bump, little, big, or even just a pebble in the road.

If it keeps getting worse, maybe you should immediately tighten your 5/8" Track Bar bolt. Tighten it down with a good breaker bar. Also, check your control arm bolts with an 11/16ths socket and breaker bar as well. Make sure these components are real tight.

Finally got the time to check and I do have a bad ball joint. I'll be fixing that within the next few weeks.
I'm also having them rebalance my tires. It just does not seem right how they balanced them.

Next up is what ball Joints to buy Yukon, Crown, etc,... any thoughts.
