Just finished putting this together this weekend. After installing my second set of tailgate hinges, I had to get the spare off of there. I can still open the tailgate halfway to get at what I need, and the carrier rotates down if I need to open it up all the way. I also installed a hitch stabilizer so there is no rattling/bouncing at all, and relocated the 3rd brake light (not shown) to the center cap of the spare.
This was a quick, inexpensive fix, while I save up (and get permission for!!) a new rear bumper/tire carrier combo.

I installed a factory radio from a 2007 Durango (almost looks like a JK radio) in mine. Then I added a Bluetooth hands free music kit and now I can stream Sirius XM through it. Not many people even notice it's not stock unless they also own a TJ. I have done a lot of other interior mods as well but nothing uncommon.
I did also put JK H13 headlights in with Sylvania Silverstar ultra bulbs. Nothing to uncommon but you don't see a lot of TJs with them yet.
A few things somewhat creative:
I re-worked my front bumper(boredom and broke mostly). Made a custom mount for my hi-lift jack, then made a mount to add a basket out back when I'm not carrying the hi-lift. Fabbed up some roll bar cross bars and a storage rack to carry gear when the top's off. Re-worked the center console to fit a slimline 8" sub. Oh, yeah, and made a trailer out of two YJ tubs to tow my gear.
I have a 2004 LJ and it has the factory sub still in the console but it makes not noise what so ever. I've replaced the head unit, I was wondering if you had a similar issue before replacing to an after market sub.
Well, I seriously doubt that either of these qualify as "custom fab", but:
Made some brackets out of aluminum angle to mount my CJ-style side mirrors to the lower windshield hinge bolt holes. The brackets that came with the mirrors mounted to the upper bolt holes and wouldn't fit on the lower ones.
Used my "redneck bending jig" to bend up some aluminum brackets to replace the broken plastic ones on my OEM fog lights.
Made some paracord door limiters today! Watched a simple YouTube video of a guy making grab handles and put a spin on it using markers as place holders, very happy with the outcome only took about 20 minutes to do!

Since I am pretty good with making things out of fiberglass, I put some speakers in my A-Pillars...
Using originals to mock up what I wanted:
After shaped and smoothed:
Vacuum bagged (latex) in the mold:
Finished product, painted:
Installed:
Also cut a hole in my hood and added a small air inlet...no reason other than I wanted to and liked the way it turned out (can always fix it if I wanted):

I have ARB bumpers on my 2006 TJ but ran into a snag when I wanted to add the Gobi Stealth roof rack. I could not use the factory Gobi towers due to the ARB bumper would not allow. So, instead of notching off the upper corners of the bumper I built a modified tower that will work with the ARB rear bumper.
