Wiring a backup camera?

I've seen several threads and videos about the various aspects of installing Lock Pick or NavTV and the placement of a backup camera, but I haven't found anything about how to route the camera wire from the outside of the vehicle to the inside.

Does this usually require drilling a hole in the body or is it best to route the cable through an existing hole (and if so, where?)?

All the cameras I've seen have RCA plugs. Does the opening have to be large enough to accommodate the RCA plug or should you cut the wire, run it through the hole, and then reattach the plug?

How about where to run the wires for a front facing camera under the body?
