The nipple is still there and its the only thing supporting the two axles in the middle where they connect. I am thinking it is the bearing, but I just cant imagine how it came out of the housing....

Sometimes the bushing in the front of the starter falls out. I guess they didn't do a good job of rebuilding it. Check to see if the copper bushing is there. This happened to me a while ago.

I have a 93 YJ and the driver side looks exactly the same. The passenger is a little different though. Seems like you have a gap on the floor where the center council sits. Mine doesnt have that.
