Also... check the drain plug on your radiator. Mine leaked a few years ago and I had to replace the drain plug which is made of plastic and subject to potential failure.

Looks like it's near your thermostat housing. Either a leak in a hose there or your thermostat housing gasket would be my first guess.

Drill out the broken screws, I did this with a regular old drill bit, it doesn't require any fancy bit. Install slightly larger diameter self-tapping...

ah... I also see it listed as "PVC Elbow". CCV stands for crankcase vent. They are both the same part.
Make sure you get new rubber seals as well....

I agree that the code is likely coincidental to the heater fan problem.
I suggest removing the three switch panel from the dash and inspecting it for melted...
