It could be that the cap was tightened correctly. If that's the problem it can take 2-3 on/off cycles driving a few miles in between for it to re-set. If it doesn't, take it in.

I haven't seen that type of issue either. I know that the fuel gauge on my 89 4.2 isn't extremely accurate. When/if I get down to 1/4 tank I know that I need to get gas right away so I don't run out....
