Another rough idle question

So for the last 6 months I have had a rough idle on my 04 LJ. When it is in neutral or park it stays at around 650 rpm but in drive at a stop it goes down to around 525-560 and the steering wheel and dash components start shaking and rattling. I have to give it gas or put it in neutral to get the RPM back to normal. I have changed the fuel pump with a Bosch, idler control, TPS sensor, cleaned the throttle body out. Spark plugs and coil pack also replaced. I am running 33 duratracs with a 3 inch lift if that matters. 112K on the odometer. It won't die when the RPM's go down but it is just plain rough.
