Could be bad inner and/or outer pinion bearings/races, or could be that the nut has backed off. Have you checked to see if the nut is tight?

Have you checked the oil, does it have metal in it? If so, probably a bearing failure. If not and gears have been installed, could be that pinion preload was not set (crush sleeve was not collapsed properly).

Pulled the carrier and pinion gear. Inner and outer pinion bearings were in bad shape. Also had a bad carrier shim that was in about 3 pieces.
