The guide that the cable runs through has a nub on the backside that centers it in a hole in the rear shoe. One of the springs goes through the hole in this guide and holds it tight to the shoe. If the guide isn't seated properly into the hole in the shoe, it will cause the cable to be loose and the adjuster arm will hang low and dig a groove into the drum. In my case, the spring was a little tweaked and wouldn't hold the guide centered in the hole.
